What is Self-Directed Care?

Self-directed home care services are a type of long-term care that helps individuals living with disabilities and people with long-term injuries stay safe and independent in their homes.

When you self-direct, you decide:

  • To stay in your home and community.
  • Who provides your care, maybe even friends or family.
  • How and when your care is done—you’re in control.

If you need help at home, self-direction may be an option for you.

Consumer Direct Care Network Montana

Consumer Direct Care Network Montana provides caregiving and nursing support to people of all ages, including older adults, children, and individuals with physical, intellectual, or developmental disabilities, so they can remain safe, healthy, and independent in their homes and communities.

Our services offer Montanans a range of responsibility levels.

Caregiving Services give you more choice and control over your care:

  • Self-Directed Services allow you, or your Personal Representative, to recruit, select, and train your caregivers who will provide activities of daily living such as dressing, medication reminders, light housekeeping, and hygiene. We support your choice and independence by managing employment-related paperwork and tasks such as payroll and taxes.
  • Traditional Agency Based Services are an option for you or your loved one if you don’t want to worry about employing your own caregiverWe hire, train, schedule, and supervise your caregivers and handle all employment and payroll-related tasks.

Nursing Services provides individualized care plans to meet your in-home, skilled care needs. Our nurses assess, monitor, educate, and provide medications and treatments as ordered by your physician. Our nurses work diligently to ensure the highest level of independence while preventing acute or chronic complications. Whether you need an hour a month or around-the-clock care, our nurses are with you every step of the way.

Personal Emergency Response Systems help you maintain independence and provide simple, fast access to help you 24 hours a day, 365 days a year. Customize your system to include your priority contacts and find peace of mind with options available at home or on the go.

Our programs are here to support you in the comfort and safety of your home and community.
