Centers for Medicare and Medicaid Services has released its Quality of Care Star Ratings for Montana.  Consumer Direct Care Network- Home Health is very proud to be Montana’s highest ranking home health provider achieving 4 out of 5 stars.  The Quality of Patient Care Star Ratings applies to 8 process and outcome quality measures:

  1. Initiated patient care in a timely manner
  2. Provided patient/caregiver drug education on all medications
  3. Better at walking or moving around
  4. Better at getting in and out of bed
  5. Better at bathing themselves
  6. Was able to engage in activity with less pain
  7. Experienced less shortness of breath

Our Home Health Services

Skilled Nursing
Licensed nurses collaborate frequently with physicians to provide medical management of acute and chronic illnesses. An individualized plan of care is developed to provide education on disease processes, treatment(s)/procedure(s), prevention strategies and coordination of care of services with physicians and other health care team members.

Physical Therapy
Your mobility and strength are important. We work to restore your mobility and strength by exercise, massage, and other methods.

Occupational Therapy
Your ability to perform activities of daily living (ADL) is evaluated. We also check your home environment for hazards or barriers, this helps, identify equipment needs.  You learn new skills or are retrained in once familiar daily activities that have been lost due to an illness or injury.

Speech Therapy
Often these disorders are the results of trauma, surgery or stroke; our nurses develop a speech therapy program to improve or restore speech/language disorders utilizing specialized therapeutic techniques and/or equipment

Medical Social Worker
A medical social worker works with you and/or your family to find services that may include short-term counseling services, referral to and coordination with community resources and assistance with living arrangements, finances, and long-term planning.

Home Health Aide/ Certified Nurses’ Aid
Services are delivered under the supervision of an RN or licensed therapist.  An HHA/CNA is assigned when there is a specific need for personal care on a part-time basis at home.
